Privacy policy

To be informed...

The reason for needing to take a detailed medical history and request personal information during the consultation process is to facilitate the very best tailored, safe and effective treatment. This also applies to any notes taken during follow up treatments. For insurance purposes I will keep this information for 10 years and destroy after this time. All information is kept in paper form and stored in a locked filing cabinet. All client records are confidential and will remain private. Please be assured that your personal information will not be passed onto third parties for marketing purposes but may be shared with other parties with your consent if considered necessary (e.g. for purposes of sharing information with another therapist you are seeing).

The therapy client data I hold may include your: name, address, phone number, email address, date of birth/age, emergency contact, GP name and contact details, relevant medical information, email correspondence (which will be deleted once i have noted the contents), invoices and session notes.

Right of access...

As a client, you have a right to see what information is being held about you. If you require such information, you are entitled to obtain copies, with no fee (as long as the request is reasonable). However, please do allow 21 days for your request to be fulfilled. Information will be given to you in a sealed envelope marked 'private and confidential' with your name clearly displayed.

Right to rectification...

Inaccurate or incomplete data can be amended at a later date. However, previous paperwork cannot be changed. I will require you to add information and note the date the information was amended with your signature consent.

Right to erasure...

As a client, you have a right to request deletion of data, however, this may be overridden for insurance purposes and with regards to medical matters.

Right to change your mind and opt out...

During the initial consultation I will provide you with a form to opt into communication via various media (as well as explaining the above). You have a right to change your mind and opt out at any time. Please contact me with what you would like to opt out and allow 14 days for me to action your request.

You also have a right to complain if you think there is a problem with the way I handle your data.
